Livonia, Michigan-based Trinity Health and Marina del Rey, California-based Heritage Provider Network announced an agreement to form a population health joint venture, called Trinity Health Partners.
Through the joint venture, the organizations plan to build care networks in specific markets using new models of care to improve coordination and integration. The venture will use Heritage's coordinated care expertise to complement Trinity's existing population health initiatives, which include clinical integration partnerships, value networks, and accountable care organizations.
