Dr. Richard Lachiver Discusses Improved Patient-Centered Medical Home Models

Dr. Richard Lachiver, Corporate Medical Director, Healthways, Franklin, TN, talks about his work with the Hawaii Medical Services Association (HMSA), which includes enhancing patient-centered medical homes (PCMH) through a well-being support model.

In addition, Dr. Lachiver talks about HMSA's drive to engage all patients through the PCMH model, the need for electronic health records to increase information exchange, and the importance of lowering hospital readmissions through behavioral change and better coordination of care.
