Jun Chiong, MD, MPH, FACC, FCCP, Associate Professor of Medicine, Loma Linda University Medical Center, states the US healthcare system does not utilize a team-based approach. Dr Chiong says that hosiptals, home healthcare workers, and private physicians act as their own entity. He also says that communication between the provider and patient is the best way to reduce 30-day readmissions for pneumonia, congestive heart failure, and COPD.
