Dr. Jove Graham Discusses How Telemonitoring Improved Patient-Case Manager Contact

Dr. Jove Graham, Research Investigator I, Geisinger Center for Health Research, Danville, PA, discusses the Post-Discharge Monitoring and Readmissions study. In this clip, Dr. Graham provides background and how this study differs from others Geisinger has conducted.

The program attempted to prevent patients from being re-admitted or going to the ER within 30 days of discharge using the telemonitoring program.
