In today’s healthcare and managed care environments, patients, physicians, and providers are faced with numerous challenges, including adverse drug events, disjointed patient care, the issues of poly-pharmacy, and ineffective lines of communication that impede the optimal care delivery. Various strategies, such as patient engagement, are being implemented to streamline patient care delivery in an effort to improve clinical outcomes while reducing healthcare expenditures and economic burden.
