AI in Breast Cancer Care Outpaces Coverage and Guidelines

Artificial intelligence (AI) adoption in the breast cancer space is moving fast, and many changes have already happened. Updated federal preventive services requirements took effect in January 2026, requiring Affordable Care Act–regulated health plans to cover recommended follow-up breast imaging without patient cost sharing. The National Comprehensive Cancer Network (NCCN) expanded its breast cancer screening recommendations in spring 2026 to include elevated AI-derived 5-year risk scores to guide recommendations for screening MRI. In 2025, Clairity Breast became the first FDA-authorized AI tool to predict 5-year risk from a routine mammogram.¹ Managed care organizations and health systems increasingly face questions not about whether the technology works, but about who qualifies, who pays, and whether imaging capacity exists to absorb what the algorithms recommend.
