Grand Rapids, Michigan — Support for type 2 diabetes (T2D) is the focus of a new plan offering from Priority Health, the nationally recognized nonprofit health benefits company. Livongo by Teladoc Health encompasses a web portal and a mobile app, and this plan supplement includes such services as 24/7 monitoring and guidance, one-on-one health coaching, and healthy habit guidance.

Other partnerships for Priority Health, with goals to optimize patient health outcomes, include joining with Arine for medication management and pulseData for plan members who have chronic kidney disease.
