Am J Manag Care. 2019;25(3):140In the Trends From the Field article, “Risk Stratification for Return Emergency Department Visits Among High-Risk Patients,” published in the August 2017 issue of The American Journal of Managed Care®, an author’s initials were incorrect in the Author Affiliations and Authorship Information sections. The initials for Wei Duan-Porter, MD, PhD, should have been “WD-P.”
Additionally, the initials of another author were inadvertently omitted from the Author Affiliations section. The affiliation of Chad Kessler, MD, should have read “Ambulatory Care Service, Durham VA Medical Center (CK), Durham, NC.”
