
FDA Clears Lerodalcibep-Liga Autoinjector, Widens LDL-C Indication
The third-gen PCKS9 inhibitor was approved in December 2025 as adjunctive treatment alongside diet and exercise for adults with hypercholesterolemia.

Label Changes With This Approval
The FDA authorized a single-use, pressure-activated autoinjector that delivers the same 300-mg/1.2-mL monthly dose as the prefilled syringe, which remains available as an administrative option.1 This autoinjector is designed to complete dosing in seconds and requires no refrigeration, preserving lerodalcibep-liga’s room-temperature stability of up to 90 days—a feature Lib Therapeutics has positioned as a differentiator from earlier PCKS9 inhibitors that require cold storage.
Adverse events were consistent with those reported with the original approval.1,2 Across trials in primary HeFH, the most common adverse reactions occurring more frequently vs placebo were nasopharyngitis, injection-site reactions, and peripheral edema. Injection-site reactions remained the most frequently cited reason for treatment discontinuation.1
Alongside the device clearance, the FDA approved revised labeling stating that cardiovascular outcomes trials have shown LDL-C reduction lowers the risk of major adverse CV events in adults treated with statins or with monoclonal antibody PCSK9 inhibitors added to statin therapy. Evan Stein, MD, PhD, founder, CEO, and chief scientific officer of Lib Therapeutics, characterized the change as an FDA acknowledgement of nearly 40 years of evidence linking LDL-C lowering to reduced CV events.

What Does the $199 Price Mean for Managed Care?
The pricing context is notable against the backdrop of PCSK9 inhibitors’ rocky access history. When alirocumab and evolocumab launched in 2015, their exorbitant pricetag prompted payers and pharmacy benefits managers to impose strict prior authorization protocols.2 A 2015 Institute for Clinical and Economic Review draft benchmark concluded those medications would need to fall to $2177 annually, an 85% discount, to meet standard cost-effectiveness thresholds.4 At $199/month, lerodalcibep-liga’s direct-to-patient cash price of $2388 per year sits close to that
Lib Therapeutics said insurance coverage is expected to expand gradually through 2027, and it plans further outreach to payers, clinicians, and patient advocacy groups to support access.1 The autoinjector is expected to be available in the US by January 2027, and the company has submitted a marketing authorization application to the European Medicines Agency, with a decision expected sometime before the end of this year, as well as a biologics license application to China, with potential approval in 2027.
