Four Major Idiopathic Pulmonary Fibrosis Advances From 2026
From taladegib to admilparant and inhaled treprostinil, here are the key 2026 advances in idiopathic pulmonary fibrosis care.
Researchers and pharmaceutical companies made major strides in idiopathic
DelveInsight released a report on pipeline insights from 2026 focused on IPF therapies, research, and approvals within the space, dating back to movements as early as 2025.1
Here are a few major advancements in the IPF space from 2026.
Investigational and Next-Gen IPF Therapies in Clinical Evaluation
In June 2026, ENV-101, AZD8965, and LTI-03 were 3 of many investigational and next-gen therapies undergoing clinical evaluation to assess their ability to delay disease progression and maintain
For example, results from a phase 2a post hoc CT analysis assessed the efficacy of ENV-101, or taladegib (Endeavor BioMedicines), a small-molecule hedgehog inhibitor, through improvements in lung volume and interstitial lung disease biomarkers. The findings were presented at the European Respiratory Society Congress 2026, held in September in Barcelona, Spain.2
The study used the Brainomix 360 e-Lung platform to assess lung volume and disease progression. With this tool, the post hoc CT analysis of 34 patients treated for 12 weeks found taladegib significantly increased lung volume (+204 mL vs –60 mL; P = .003) and reduced total disease extent (–2.9% vs +1.0%; P = .012) compared with placebo.2
FDA Designations and sNDA Acceptances in IPF
In August 2026, Rein Therapeutics announced the FDA's decision to grant Fast Track designation to its investigational, first-in-class therapy LTI-03. The drug has also received orphan drug designation. The inhaled peptide therapy derived from caveolin-1 biology, a key regulator of fibrotic signaling, is designed to inhibit scarring while preserving cells critical for tissue repair and regeneration.3 The phase 2 RENEW (
In September 2026, United Therapeutics announced that the FDA accepted its supplemental new drug application (sNDA) for inhaled, nebulized treprostinil (Tyvaso), with the FDA's review expected to be completed in late April 2027. The sNDA was supported by positive data from the phase 3 TETON-1 (
In a combined analysis, the trials met the primary end point of absolute change in forced vital capacity (FVC) from baseline to 52 weeks, with a 111.8-mL difference favoring treprostinil over placebo. Patients receiving treprostinil also experienced a reduced risk of clinical worsening and acute exacerbation of IPF.4
Pivotal Phase 3 IPF Clinical Trial Results
On October 12, 2026, Bristol Myers Squibb reported positive topline findings from the phase 3 ALOFT-IPF (
The 52-week study randomly assigned approximately 1255 adults to receive 60 mg or 120 mg of admilparant twice daily or a placebo and compared their safety, efficacy, and tolerability. The data showed the 60-mg dose significantly improved absolute change in FVC compared with placebo; the 120-mg dose slowed FVC decline, but the difference was not statistically significant.5
"Taken together, these topline data reinforce the profile of admilparant as a promising, clinically meaningful investigational therapy for IPF—one that reflects the innovation of targeting the LPA1 pathway as a first-in-class approach," Cristian Massacesi, MD, executive vice president, chief medical officer, and head of development at BMS, said in a press release.5
Overall, 2026 marks a significant year for IPF therapies, research, and potential new options for
