
Understanding the Immune Pathophysiology Driving Chronic Hepatitis B

Welcome back to another AJMC Insights series. In "Understanding the Immune Pathophysiology Driving Chronic Hepatitis B," moderator Raymond Chung, MD is joined by Douglas Dieterich, MD to open the series on chronic hepatitis B, beginning with the immune mechanisms that allow the virus to persist.
Dr. Raymond Chung opens the series by outlining its scope: identifying the most clinically significant unmet needs in current chronic hepatitis B management, reviewing recent clinical trial data on emerging antiviral agents, and describing how health-system analytics can prioritize quality-improvement efforts and patient access to therapy. He then asks Dr. Douglas Dieterich to explain the disease's pathophysiology for an audience that may understand hepatitis B primarily as a viral infection, and why its immune dimension matters so much for diagnosis and treatment.
Dr. Dieterich explains that Baruch Blumberg's original discovery of the virus revealed a defining feature: a massive excess of hepatitis B surface antigen. He describes how these enormous quantities of surface antigen effectively overwhelm the immune system, suppressing its ability to mount an effective response. He notes that this immunologic mechanism went underappreciated for decades and has only become clear recently, as researchers have worked toward curing the disease rather than simply suppressing it.
He explains that new therapies now in development are designed around specific surface antigen thresholds, or set points, that appear to predict a patient's chances of clearing the virus. This shift has changed how clinicians think about immune function in hepatitis B, moving the field beyond a purely virologic framework. Dr. Dieterich adds that suppressing viral replication with existing antiviral therapy remains essential in the background, since lowering replication likely helps the immune system regain some capacity to respond. Bringing surface antigen down to a low enough level, he says, is what ultimately creates the opportunity to consider clearing the virus from the body entirely, rather than simply controlling it indefinitely.
